Our Assisted Living is designed for seniors who want a residential setting, but require some help with tasks of daily living in areas such as bathing, dressing, and dining.
The Forum at Desert Harbor offers a wealth of Assisted Living services to cater to the unique needs of every individual. Each Wellness Plan is individualized.
We meet with residents and family to conduct an assessment—prior to move-in— to understand the need for assistance and support. The assessment helps determine the appropriate care, preserving each resident’s dignity while encouraging independence and freedom of choice.
Skilled Nursing
The Forum at Desert Harbor Skilled Nursing program places a premium on preserving our residents’ dignity and self-respect while providing nursing and rehabilitative care.
A team of licensed professionals works with residents, families, and their physicians to develop individualized plans of service.
Throughout each day, our residents are encouraged to enjoy a variety of activities that reflect their interests and former occupations.
Long-term – The Forum at Desert Harbor offers a home-like setting and 24-hour professional nursing services to provide a comfortable environment for seniors in need of nursing care.
Short-term/Rehabilitation – Physical, occupational,and speech therapies are available for our residents. Therapists will work with the resident toward a goal of becoming as independent as possible, in as timely a manner as possible. Our residents have access to specialized equipment in our rehabilitation area.
Hospice Care
At the Forum at Desert Harbor, our caring team works with a network of preferred providers making it possible for our residents who need hospice care to stay at our community and have their needs met in the most comfortable, home-like setting.
